CHP’s clinical care teams work to provide quality primary care for men, women and children with a commitment that includes dignity and kindness. CHP engages both bilingual staff members and interpreting services to communicate and provide responsive care for our diverse patient population.
CHP provides and or links patients to primary medical, dental, behavioral health and enabling services. Medical services emphasize primary care across the patient’s life span, including family medicine, internal medicine, infectious disease management, pediatric care, OB/GYN services, prenatal and postpartum coordinated care with labor and delivery, immunizations and preventative screenings, and chronic care.
