“For ten years, CHP has cared for and supported me and my family. When I was diagnosed with diabetes, my A1C was above 10. With help from the CHP team and a proper diet plan, I lowered it to 6.3 and have kept it there.
I’m grateful that CHP is easy to reach, including same-day appointments when I need urgent care. Their care and professionalism have made a real difference in my life and my family’s health.

A Wonderful Experience for Me and My Community
“A couple of weeks ago I was having an awful ear infection. I’m a patient at the CHP Rantoul satellite site and I went there for support. Even though there is not a provider at the site, all the staff, provided me the support that I needed.
They coordinated my transportation in their new patient passenger van to the Kankakee location where I received treatment and was able to feel so much better. Having that new transportation van is such good news in these kinds of emergencies. The staff of CHP surely go above and beyond to provide support to all of us.
