Welcome to CHP’s MyCare patient portal.

MyCare is a free service that provides secure, online access to your health information and medical health records. To access MyCare, you need access to the Internet, an email address and personal information to help identify yourself.

MyCare lets you:

• Receive secure messages from your care team
• Get medical appointment reminders
• View visit notes and lab results
• Complete online check-in
• Submit prescription refill requests and MORE!

To create a patient portal account, please speak to the staff at your local clinic site.
