New Provider Announcement: Welcoming Kayla Jones, Family Nurse Practitioner

We are pleased to announce that Kayla Jones, DNP-FNP-BC, has joined Community Health Partnership of Illinois at the CHP Kankakee Health Center. She brings her full-practice authority to CHP.

 

She is a Board-certified Family Nurse Practitioner with experience in community and public health and ambulatory outpatient environments delivering value-based care.
