Payment and insurance information: 

We accept most third-party insurance, including all Illinois Managed Care health plans, Medicare and Medicaid insurance plans. If you have no income and no insurance, no problem. A sliding fee scale program is in place and charges are based on your family size and income. For more information, see our Paying for Services page.

Before your visit:

Gather and bring your insurance card(s) if you have coverage, all the medicines you are taking (if it is your first appointment), and immunization records and any other health documents.

If you need to cancel your visit, please call the clinic at least 24 hours before your visit.

Services at the CHP Champaign Health Center:

MEDICAL CARE:

  • Mobile medical unit, bringing primary care services directly to agricultural workers’ housing complexes and other locations
  • Primary care services for all ages
  • Well-child care (regular checkups, school physicals)
  • Lab and basic diagnostic services
  • Screening for diabetes, hypertension, colon cancer, cervical cancer and other health conditions
  • Chronic disease management and support for patients with diabetes, hypertension, asthma and other chronic conditions
  • Women’s health services, including breast cancer screening and Pap smears
  • Care coordination


BEHAVIORAL HEALTH CARE

SUPPORT SERVICES:

  • Assistance paying for prescription medicines
  • Bilingual (Spanish-English) interpretation, care coordination and health education


AFTER-HOURS COVERAGE:

  • CHP patients will receive at the end of their visits a summary which includes instructions for after hour coverages
  • If you have a medical need that cannot wait until tomorrow, please dial 630-962-4354 to connect to the after-hours nurse 

  • In the case of a life-threatening emergency, please call 911
