For more than 50 years, Community Health Partners of Illinois (CHP) has provided high-quality healthcare to medically underserved individuals in rural Illinois communities. Last year alone, CHP served more than 10,000 individuals across its clinics located in Aurora, Harvard, Kankakee, Mendota, Champaign and Princeville.
Your gift to CHP helps us provide compassionate, respectful and accessible medical, dental and behavioral health services to help our patients attain their best health regardless of their ability to pay. While all of our clinics serve the entire community, we continue to have a commitment to our migrant and seasonal farm workers and their families to ensure they have the healthcare they need and deserve.
During the current COVID-19 health crisis, your support is more critical than ever before. While the need for our medical and dental services is always high, we have seen a surge in the need for counseling services and behavioral health. We rely on the support of caring and generous friends like you to help us raise additional funding.

To donate by mail, please write in the note section on your check the area you would like to earmark your gift:
