Eleace Sawyers, MAcc, MPA, CHC
Chief Executive Director
Eleace is an accomplished healthcare executive with over 19 years of experience in the healthcare industry. Eleace is the Chief Executive Officer for Community Health Partnership of Illinois (“CHP”). As CEO, she oversees an interdisciplinary staff, providing primary preventative healthcare for six (6) health centers from more than seven counties throughout Illinois.
She is the proud recipient of numerous awards, including the 2017 Who’s Who in Black Cleveland “Game Changer” award, 2016 Community Development Leadership and Supporter Award (Diaspora Arts Coalition), the 2014 Florida Association of Community Health Centers Partnership Award, and 2012 South Florida’s 100 Most Accomplished Caribbean Americans Award.
Eleace serves on many community and professional boards. She is a graduate of Florida International University, where she earned a master’s degree in public administration. She also holds a master’s degree in accountancy from Millsaps College, located in Jackson, Mississippi. She is a certified member of the Health Care Compliance Association and a member of the American College of Healthcare Executives.
Pitamber Persaud, M.D.
Dr. Persaud has more than 16 years with Community Health Partnership of Illinois (“CHP”) working part-time as a board-certified internal medicine physician. In 2020, Dr. Persaud accepted the role of Medical Director, directly supervising medical, clinical and behavioral health services.
Daria Sztaba, MAcc, B.A.
Daria Sztaba is an experienced Director of Finance with a demonstrated history of working in the not-for-profit healthcare and government relations industry. Daria earned both bachelor’s and master’s degrees in accounting from Purdue University. Daria brings over 18 years of comprehensive not-for-profit experience in financial reporting, grants management, budgeting and accounting operations. She has more than five years of experience working for Federally Qualified Health Centers in the Chicago area.
Jina Ramirez, B.A.
Jina Ramirez is the Operations Director at CHP. Jina has worked with CHP since 2005 in various roles, including Clinic Director, Regional Outreach Coordinator, HIV/STI Test Counselor, Migrant Health Advocate and Promotores de Salud Program Coordinator.
Maria Villagomez, MPH
Maria is the Quality & Risk Management Director at CHP. Maria began as a Health Information Technology Specialist in 2014 and was promoted to Quality & Risk Management Director in 2016. As the Quality & Risk Management Director, Maria supports CHP’s clinical performance goals and risk management initiatives. Maria holds a Master of Public Health degree from Benedictine University and is completing an associate degree in nursing currently. She is passionate about using evidence-based practices to improve patient health outcomes in vulnerable populations.
Camesha Richardson, MBA, SPHR
Human Resources Director
Camesha Richardson is the Human Resources Director at CHP. Camesha worked in various HR roles as a Human Resources Business Partner for United Ground Express and an HR Analyst with the University of Chicago. Camesha is a graduate of the University of Illinois Urbana-Champaign, where she earned a Master of Business Administration. She also is a graduate of Southern Illinois University Edwardsville, where she earned a bachelor’s degree in human resource management. Camesha is a certified Senior Professional in Human Resources (SPHR).
Kimberlee Greif, LPN
Kimberly Greif has served as Nursing Director at Community Health Partnership of Illinois (“CHP”) since 2020. Prior to serving as Nursing Director, Kimberly worked for CHP as Clinic Director of the Harvard Health Center.
Heather A. Reffett, MPA, CPM
Director of Strategic Initiatives
Heather leads CHP’s strategic initiatives. She is a Certified Public Manager with distinction in municipal management, and she has expertise serving as a consultant to county leadership. Heather holds a Master of Public Administration degree from George Mason University with a concentration in state and local government.
Interim Development Director
Susanna brings more than 30 years of fundraising experience to CHP. She has a strong background in raising funds for health care systems, community health clinics and other social service organizations across Chicagoland.
Susanna is an active volunteer with several local organizations and holds a BA from Purdue University.
