Community Health Partnership of Illinois (CHP) provides high-quality, patient-centered, comprehensive primary care in a setting that is welcoming and responsive to the cultural, language and health needs of the communities we serve.
We are committed to providing healthcare with dignity, kindness and fairness. CHP’s commitment to diversity is reflected in its leadership and patients. CHP empowers the uninsured, underserved, and Migrant and Seasonal Agricultural Workers (MSAW) to attain their best health. Our network is comprised of multiple medical and dental clinics as well as outreach sites across rural and urban communities in northern and central Illinois.
Our team includes providers who specialize in family medicine, internal medicine, infectious disease, psychiatry, OB/GYN and pediatrics.
